SUNDAY:
ARSENAL V EVERTON
All Arsenal have to do here is win and they win the title, hoping that Man City slip up in their game against West Ham.
Even if the Gunners don't win the title, it's still been another good season of progress. They're a much better team than they were last season and I expect them to go again next season.
Everton will want to make it difficult for Arsenal here in an attempt to stop them from winning the title.
But if think the Gunners will have enough firepower up top and solidity in defence to come away with the points here.
PREDICTION: 2-0
BRENTFORD V NEWCASTLE
This could well be the last time we see Ivan Toney in a Brentford shirt as he is more than likely to leave the club this summer.
Toney is not in good form at the moment but the Bees did show signs of life without him against Bournemouth with Wissa and Mbeumo both scoring and assisting each other. That will be an encouraging sign for Thomas Frank's side heading into next season.
Newcastle have struggled away from home for most of the season and they suffered another defeat on the road against Man United in midweek.
But I think they'll bounce back here though as I think they'll have enough quality to win the game and seal European football for another season.
PREDICTION: 1-3
BRIGHTON V MAN UNITED
Brighton did the double over United last season and they have the chance to do the same again here having won the reverse fixture back in September.
Erik Ten Hag's first Premier League game in charge of Man United was against Brighton and his last one could be also.
But their form this season has been very inconsistent and are set to finish outside the European places.
I can see Brighton make sure of that as they're a more convincing team to watch and a team that play with more energy.
PREDICTION: 3-1
BURNLEY V NOTTINGHAM FOREST
This game might not be as big a game for Burnley as it might've been a few weeks ago and that is because of their relegation being sealed last weekend.
But Vincent Kompany will still want his side to put in a performance here so they can end the season on a good note.
Nottingham Forest are pretty much safe as they have a far superior goal difference to Luton. But they still need a result here to guarantee it.
Their away form since they've been in the Premier League has been terrible but they did win their last game on the road against Sheffield United.
I think they'll get another rare win on the road in this one to secure their safety against a Burnley side who have been hopeless at home all season.
PREDICTION: 0-3
CHELSEA V BOURNEMOUTH
Chelsea could finish 5th if they win this game which would be absolutely remarkable give the rollercoaster of a season they've had.
They're ending the season very strongly and are playing more like team than they have done for most of the season.
Mauricio Pochettino has had his fair share of criticism this season but he deserves a lot of credit for turning his sides' season around.
Bournemouth were one of my relegation favourites at the start of the season and their poor start vindicated that. But from November onwards, they've proved me wrong and Andoni Iraola deserves a lot credit for the job he's done given the slow start he had.
They've certainly been more clinical this season and that is because they've had a prolific finisher up top in Dominic Solanke who has proved a lot of people including myself wrong this season.
Chelsea have been in decent form at home this season and I can see them keep up that form with another victory here, which will secure European football for them.7
PREDICTION: 3-1
CRYSTAL PALACE V ASTON VILLA
Crystal Palace's form over the last month has arguably been their best spell of the entire season which has mostly been underwhelming.
The future definitely looks bright under Oliver Glasner, but he will have a big task on his hands in the summer in trying to keep hold of his two best players in Michael Olise and Eberechi Eze.
Aston Villa celebrated Champions League qualification on Tuesday with Spurs' defeat to Man City sealing their place.
Unai Emery deserves a huge amount of credit for the job he's done and in my opinion, deserves to win the manager of the season award. He's taken Villa onto the next level.
There will definitely be goals here as both sides have some good attacking players. Palace will definitely cause Villa problems but I think the away side will have enough in them to come away with a point.
PREDICTION: 2-2
LIVERPOOL V WOLVES
Liverpool will be determined to end the season on a high with this game being Jurgen Klopp's final match in charge.
I hope both sets of players give him a guard of honour before the match because of how good a manager he has been both for Liverpool and the Premier League.
Wolves have had a promising season given the state they were in before the season started. But they're not ending it in the best of form, having won just once in their last eight games.
Liverpool will be more up for it here for obvious reasons and I think their hunger will get them over the line and make it a winning farewell for Klopp.
PREDICTION: 3-1
LUTON V FULHAM
Luton will need a 12 goal deficit to go in their favour if they're to stay up but unfortunately for them, I don't see it happening.
The players appeared to accept their fate after their loss at West Ham last weekend as their body language showed. Rob Edwards will need to pick up his players ahead of this one as they do still have a chance. But a very small one.
Fulham haven't been great away from home all season and are not ending the season in particularly good form either. They've only won one of their last seven games.
But I do think they'll win this one as I think they'll have enough to send a low on confidence Luton side back to the Championship.
PREDICTION: 0-2
MAN CITY V WEST HAM
Man City head into the final day in pole position to win the title after winning their game in hand against Spurs in midweek. They had substitute keeper Stefan Ortega to thank for in that game as his crucial saves kept them in the game.
All they need to do here is win and they win a fourth successive Premier League title, something that has never been done before.
West Ham boss David Moyes will want go out on a high note here and his players will be determined to give him the send off he deserves.
But I don't see them doing that as I think City will be too strong for them. I can see their quality being too much for the Hammers.
PREDICTION: 3-0
SHEFFIELD UNITED V TOTTENHAM
Sheffield United will be glad to see the back of this season once this game has finished as this has been one rotten campaign for them.
They've been rock bottom of the league pretty much all season and also have the worst defence in Premier League history, conceding 101 goals.
Spurs saw their top four hopes end for good following their loss to Man City in midweek and whilst they'll feel disappointed for not finishing there, they've still had a decent campaign. Especially after losing their best player in the summer.
Ange Postecoglou will want his side to end the season with a strong performance here and I think he'll get one. I can see his side winning very comfortably here as the Blades are low on confidence and going down with a whimper.
PREDICTION: 0-5
